O-Desmethyltramadol (INN: Desmetramadol, short form O-DSMT) is the actually effective breakdown product of the painkiller Tramadol.The liver CYP2D6 splits off a methyl group - like with codeine - which is why people with a weak CYP2D6 variant experience less pain relief from tramadol.Because O-DSMT itself does not require any further conversion, it remains fully effective in them and can in principle be used directly.

rnrnMechanism of actionrnThe (+)-enantiomer is a complete, G protein-mediated µ-opioid receptor agonist with only weak affinity for δ and κ receptors.The (−)-enantiomer additionally inhibits norepinephrine reuptake without blocking the serotonin transporters.Together, two synergistic signaling pathways result: µ-opioid-mediated analgesia and noradrenergic pain modulation - useful in neuropathic or complex pain syndromes, but also with a higher risk of interaction.
rnIn addition, O-DSMT antagonizes the 5-HT₂C serotonin receptor (competitive and in therapeutically relevant concentrations).This blockade lifts the receptor's inhibitory control over dopaminergic and noradrenergic core areas (prefrontal cortex, striatum, nucleus accumbens), which could mediate mood elevation and possible antidepressant effects - comparable to agomelatine or maprotiline.Although O-DSMT would theoretically represent a more selective antidepressant with fewer side effects, corresponding clinical programs were not pursued further.
